Registration options:

• Online: Register Now! (You won't want to miss the CVC Advantage Pricing that ends at Midnight (CT) on August 1.) 
• By fax or mail: The registration form will be available on May 18, 2012.  At that time you can complete and mail all six pages and your check (U.S. dollars only), money order, or credit card informaiton to: CVC San Diego, Advanstar Communications Inc., Attn: Cash Department, 131 W. First Street, Duluth, MN 55802.

The registration form is for one attendee and guests. Use a separate registration form for each attendee, making copies as needed. Everyone attending CVC San Diego and requesting hotel accommodations must be registered. Write your name as you wish it to appear on your badge and in the registration roster. Guests only have access to the Exhibit Hall. Hospital team members are not considered guests. All veterinarians, veterinary technicians, practice managers, and team members must register and pay registration fees. Exhibit Hall-only badges for team members may be purchased ($45 each on site). Exhibit Hall-only badges are available on site for $45 for veterinarians, technicians, practice managers, and team members who are affiliated with a practice and/or not working for an industry company.

Registrations made using a purchase order will be accepted in lieu of payment to hold your place. All payments must be received before the start of the conference.

Wet Labs & Workshops
Wet Labs and Workshops require an additional (nontransferable) registration fee. CVC reserves the right to cancel any lab or workshop that does not meet minimum registrations by. If the lab is cancelled by CVC, registrants will be notified and all paid fees credited or refunded.

All cancellations of a lab, workshop, or registration by the registrant must be submitted in writing for a credit or refund (minus a $50 processing fee). No requests for refunds or changes will be accepted after October 31, 2012.

Child Care
Licensed child-care services will be offered  in the Convention Center. Sign up for child care on the registration form and indicate the age of each child. You will receive information and forms for each registered child. Child care is limited and confirmed on a first-come, first-served basis. Cancellations must be made in writing and received no later than October 31, 2012. No cancellations will be accepted after this date.

Registration Changes or Cancellations
